    If paying by check, please make check payable to HCCMO and send it to 3201 E. Colonial Dr., Suite A20, Orlando, FL 32803. Full payment due with signed contract & rules and regulations form no later than Monday, October 25th, 2021



    IMPORTANT: To secure your booth(s), Expo Agreement documents (Registration Agreement, Rules and Regulations and Agreement for Exhibit Space) must be filled out and signed by company representative and payment must be received in full. NO booths will be assigned or reserved without full payment and signed agreement.
    ACCEPTANCE OF TERMS
    By signing this agreement, I accept that I have read and understood the term and conditions of this contract including the Rules and Regulations and Agreement for Exhibit Space, Accident Waiver and Release of Liability and Permission to be Photographed Waiver.
